H2 Dermatology uses all digital record keeping. We use an Electronic Health Record (EHR) that improves both confidentiality as well as efficiency within our practice. We also have the utmost respect for patients' time. For this reason, we ask that all patients have their New Patient Information paperwork, Consent for Treatment and Clinical Procedures, Covid-19 Consent, HIPAA/Informed Consent, and Consent to Treat a Minor (only if patient is under 18 years of age and plans to be seen without a parent or guardian) forms filled out BEFORE the start of their appointment. This can be done one of three ways:
1. Our preferred option is for all new patients to use our Patient Portal. Our Patient Portal can be located under the "Patient Resource" tab. If you have questions about the Patient Portal, please contact us for assistance.
2. Our second preference is that patients show up to their appointment AT LEAST 15 minutes early to digitally enter their information via our Patient Kiosk.
3. Your third, and least preferred option, is to use the links below to fill out your paperwork by hand prior to your arrival. That information will then have to be digitally uploaded to your health record before being seen.
